A 42-year-old woman has three small enhancing brain lesions…
A  Osimertinib

Explanation
The key facts are the EGFR exon 19 deletion, the asymptomatic small-volume brain disease, and the patient being neurologically intact.

I authored the FRCS(SN) question bank I wish I’d had.

I sat the FRCS (SN) in 2025. My preparation was scattered with resources that weren’t tailor-made for the modern day JCIE exam. Books catered to mainly international exams, used outdated terminology and rewarded recall, not understanding. I decided to develop a platform from the ground-up which reflected the real exam.

Our questions are written in SBA format according to NBME standards, developed around clinical reasoning, with tiered difficulty settings to stretch all levels of candidates.

Every distractor is individually defensible. If a question can be answered by recognising a single line of guidance, we rewrite it. The exam tests judgement between reasonable options - so we do too.
